As a Supports Broker, you will assist individuals and their families in navigating and managing self-directed services under Pennsylvania’s Office of Developmental Programs (ODP) regulations. This role focuses on person-centered planning, service coordination, advocacy, and ensuring compliance with ODP guidelines, enabling individuals to exercise choice and control over their supports.
Key Responsibilities
Person-Centered Planning & Individual Support Plan (ISP) Development
- Conduct comprehensive assessments to understand the strengths, needs, and goals of each individual.
- Facilitate person-centered planning meetings, ensuring the individual’s preferences, choices, and voice are at the center of all decision-making.
- Collaborate with the individual, their family, and the Interdisciplinary Team (IDT) to develop and implement an Individual Support Plan (ISP) that aligns with self-directed services.
Service Coordination & Support Management
- Assist individuals in identifying, selecting, and coordinating service providers and other supports that align with their needs.
- Guide individuals in managing their service budgets, ensuring resources are used effectively to support their goals and independence.
- Monitor the delivery of services, ensuring they are provided in accordance with the ISP and ODP regulations.
Advocacy, Empowerment & Education
- Educate individuals and their families on self-direction, available services, and their rights under ODP guidelines.
- Advocate for individuals, ensuring they have the knowledge and confidence to make informed decisions about their support services.
- Provide training and guidance on hiring, managing, and evaluating direct support staff.
Monitoring, Evaluation & Compliance
- Conduct regular reviews and updates of the ISP to reflect changes in the individual’s needs, goals, or circumstances.
- Ensure compliance with all ODP policies, regulations, and documentation requirements, including service utilization, reporting, and record-keeping.
- Monitor the quality and effectiveness of services, identifying any gaps or areas for improvement and addressing them accordingly.
Documentation & Reporting
- Maintain accurate, up-to-date records of all services provided, progress reports, and ISP updates.
- Submit required reports to regulatory agencies and funding sources in a timely manner.
- Follow confidentiality policies and HIPAA regulations in handling personal and medical information.
Qualifications & Requirements
Education & Experience
- High school diploma or GED required; Associate’s or Bachelor’s degree in human services, social work, psychology, or a related field preferred.
- ODP certification as a Supports Broker is required (or willingness to obtain certification before employment).
- Minimum of 2 years of experience working with individuals with IDD, preferably in case management, service coordination, or self-directed supports.
- Knowledge of ODP waiver programs, self-directed service models, and person-centered planning principles.
Skills & Abilities
- Strong interpersonal and communication skills, both verbal and written.
- Ability to work independently while effectively collaborating with individuals, families, and service providers.
- Excellent organizational and time management skills, with attention to detail in documentation and compliance.
- Proficiency in Microsoft Office, data management systems, and electronic documentation tools.
- Familiarity with local resources and service providers supporting individuals with IDD.
Certifications & Licenses
- ODP certification as a Supports Broker (required).
- Valid driver’s license and reliable transportation (travel within the community and to individual homes may be required).
- Certification in Person-Centered Planning or related areas is a plus.
Physical & Travel Requirements
- Ability to perform the essential functions of the job with or without reasonable accommodation.
- Willingness to travel within the community to conduct home visits, attend meetings, and provide on-site support.
