Physical Therapist (PT)

As a Physical Therapist (PT), you will play a vital role in assessing, treating, and enhancing functional movement for individuals receiving services in residential, community, and home-based settings. Your work will align with Pennsylvania’s Office of Developmental Programs (ODP) regulations and best practices in disability-focused therapy.

 

Key Responsibilities

 

Comprehensive Assessment & Individualized Therapy Planning

  • Conduct thorough evaluations of individuals’ musculoskeletal, neurological, and functional mobility to assess strength, flexibility, balance, coordination, and motor skills.
  • Develop individualized treatment plans based on the individual’s functional goals, needs, and abilities, ensuring alignment with their Individual Support Plan (ISP).
  • Utilize adaptive assessment tools and techniques tailored to individuals with IDD and mobility challenges.

 

Therapeutic Interventions & Functional Training

  • Implement evidence-based physical therapy interventions such as therapeutic exercises, gait training, neuromuscular re-education, and balance training.
  • Provide manual therapy, assistive device training, and positioning strategies to improve mobility, posture, and pain management.
  • Incorporate adaptive equipment and assistive technology to enhance individuals’ independence and participation in daily activities.
  • Work with Direct Support Professionals (DSPs) and family caregivers to ensure proper implementation of therapeutic activities in daily routines.

 

Education & Home Exercise Programs

  • Educate individuals, families, and support staff on proper body mechanics, mobility techniques, and injury prevention strategies.
  • Develop individualized home exercise programs to promote continued progress, safety, and self-management.
  • Provide training on proper use of mobility aids, orthotics, and adaptive equipment to enhance community participation.

Progress Monitoring & Outcome Measurement

  • Conduct regular reassessments to track progress, adjust therapy goals, and modify interventions as needed.
  • Utilize objective outcome measures to assess the effectiveness of interventions and maintain data-driven treatment approaches.
  • Maintain accurate, timely documentation of assessments, treatment plans, progress notes, and discharge summaries, following ODP guidelines and regulatory standards.

Interdisciplinary Collaboration & Advocacy

  • Work closely with Occupational Therapists (OTs), Speech-Language Pathologists (SLPs), Behavior Specialists, and Support Coordinators to ensure a holistic, person-centered approach to care.
  • Collaborate with families, caregivers, and DSPs to integrate therapeutic strategies into daily routines.
  • Advocate for individuals to access community-based recreation, fitness programs, and mobility supports to enhance inclusion and participation.

Compliance, Professional Development & Leadership

  • Adhere to all ODP policies, HIPAA regulations, and best practices in disability services.
  • Engage in ongoing professional development to stay informed on advancements in therapy, assistive technology, and IDD-focused interventions.
  • Provide training and mentorship to staff and caregivers on mobility, safety, and therapeutic techniques.

 

Qualifications & Requirements

 

Education & Licensure

  • Doctor of Physical Therapy (DPT) from an accredited program required.
  • Current Pennsylvania Physical Therapist (PT) license or eligibility for licensure.
  • CPR certification (required); additional specialty certifications in neurology, orthopedics, pediatrics, or geriatrics preferred.

 

Experience & Skills

  • Experience working with individuals with IDD or neurological conditions preferred.
  • Strong clinical reasoning, assessment, and treatment planning skills.
  • Ability to develop and implement person-centered care plans aligned with ISP goals.
  • Familiarity with adaptive equipment, mobility aids, and assistive technology.
  • Excellent interpersonal, communication, and teamwork skills to effectively collaborate with interdisciplinary teams.
  • Commitment to empowering individuals to achieve their highest level of independence.

Additional Requirements

  • Valid driver’s license and reliable transportation (as travel to community and residential sites may be required).
  • Ability to work independently and within a multidisciplinary team in various environments.
