As a Licensed Practical Nurse (LPN), you will play a vital role in providing high-quality nursing care to individuals under the direction of registered nurses (RNs) and other healthcare professionals. Your responsibilities will encompass a wide range of clinical and administrative tasks aimed at promoting the health, safety, and well-being of individuals across various healthcare settings.

Key Responsibilities:

1. Individual Care:
   • Perform comprehensive assessments of individual’s health status, including vital signs, medical history, and current symptoms.
   • Administer medications, injections, and treatments as prescribed by physicians or advanced practice providers, ensuring accuracy and adherence to medication administration protocols.
   • Monitor individuals’ condition and response to treatments, promptly reporting any changes or concerns to the supervising RN or healthcare team.
   • Provide wound care, dressing changes, and other routine nursing procedures as needed to promote healing and prevent complications.
2. Care Planning and Documentation:
   • Assist in the development and implementation of individualized care plans based on individuals’ needs, preferences, and treatment goals.
   • Document all nursing interventions, assessments, and observations accurately and timely in individuals’ medical records, maintaining confidentiality and compliance with healthcare regulations.
   • Collaborate with interdisciplinary team members, including physicians, therapists, and social workers, to coordinate individual care and ensure continuity of services.
3. Individual Education and Support:
   • Educate individuals and their families about their health conditions, treatment plans, and self-care strategies, empowering them to actively participate in their own care.
   • Provide emotional support and guidance to individuals and families coping with illness, injury, or chronic health conditions, demonstrating empathy, compassion, and cultural sensitivity.
4. Quality Assurance and Safety:
   • Adhere to established nursing standards, protocols, and policies to promote individual safety, infection control, and regulatory compliance.
   • Participate in quality improvement initiatives, audits, and risk management activities to identify areas for enhancement and ensure optimal individual outcomes.
5. Professional Development:
   • Pursue opportunities for continuing education, training, and professional development to enhance clinical knowledge, skills, and competencies.
   • Maintain licensure and certification requirements, staying informed about changes in nursing practice standards and regulations.

Qualifications:

• Graduate of an accredited practical nursing program.
• Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
• CPR certification and other relevant certifications as required by the healthcare facility.
• Strong clinical assessment skills and ability to make sound nursing judgments.
• Excellent communication and interpersonal skills, with the ability to collaborate effectively with individuals, families, and healthcare team members.
• Ability to work independently and prioritize tasks in a fast-paced environment.
